1.
The "required field" check is a setting to make sure people need to fill in the specific field in the booking form.
If you select one of the radio buttons and complete the booking you can display the selected value in your email template by adding the following shortcode to Appointments -> Settings -> Emails:
%customfields%
If you mean that the selected value of one of the radio buttons will be unchecked after you reload the page, this is how the Booked plugin is coded and supposed to work. Added or selected data in the booking modal will be removed/empty is you reload the page.
2.
The Booked plugin don't support custom fields for the register and login forms. I really hope they will include this feature in their plugin because it make a lot of sense. We're not the developer of this plugin so I can't help with this unfortunately.
For now the only option to add custom fields to a register/login form is by using the the RegistrationMagic plugin for this (so you don't use the [booked-login] shortcode anymore).
This plugin provides an login and register form out of the box that you can add to any page by using a shortcode as well. Basically you add the default login form shortcode to a page and redirect the form to the [booked-account] page after logging in, the same could be done for the registration form.
The checkbox does, indeed, not save after selecting it. I just tested it on my local theme installation and can confirm this issue.
After checking the comment section of this plugin on CodeCanyon somebody forwarded this issue to their support and they came up with the following (temporary) solution:
Unfortunately I can't give an answer on that questions because we're not the developers of this plugin, we only bundle it with our theme.
The issue is known at their support team so I assume they forward this to their developers. The only I can hope is that they fix this as soon as possible.

You can install and enable the jQuery Migration Helper plugin that should fix the issue.
