Have you added the receiver email address within the 'Mail' tab of this form?
If you have added the receiver email address but the form still doesn't send you need to install the WP Mail SMTP plugin as there is a issue with the email server. Unfortunately this is a common issue with WordPress and can be fixed by installing and setting up the plugin. Please follow the steps in this article to setup the plugin:
