Any time I submit the schedule appointment form, it gives validation error if I input the date format. Without the date of appointment, the form gets delivered with no date information. But if I input the date format it shows validation error with date format invalid. The error is from the date, every other thing works fine.
I just tested the forms a couple times and did not notice any validation issue. Please see this screencast how I send the form http://quick.as/jga9HvlPV. Did I miss something?
